You can create an evaluation form to assess employees' job performance. After you create a form by naming it and giving it a description, you add tabs, called standards, for your core competencies. In each standard, you add sections, called indicators, to break the core competency into subcategories. For example, in the image below, the standard Lesson Planning has the indicator Curriculum and Planning.

In your indicators, you add the benchmarks, called elements, used to evaluate job performance. Lastly, within each element, you add the responses, called descriptors, that the evaluator can select for each benchmark. In the image below, the element Subject Matter Knowledge has the descriptors Needs Improvement, Meets Expectations, and Exceeds Expectations.

The tasks involved in the creation of a form must be done in the following order:

  1. Create a Form
  2. Add Tabs to a Form
  3. Add Sections to a Form
  4. Add Benchmarks to a Form
  5. Add Responses to a Form
